Program Manager
JOB DESCRIPTION
REPORTS TO: Administratively to the CCFSA Director of Programs
TIME ALLOCATION: 100% SCY Program
DIRECTLY SUPERVISES: This position directly supervises the CCFSA member agency SCY Community Health Workers (CHW)
JOB PURPOSE: The SCY Project Manager provides programmatic oversight of CCFSA SCY Community Health Workers (CHW). The Project Manager provides leadership to assure fulfillment of the program’s mission through achieving specific outcomes for the clients served. The Program Manager is primarily responsible for daily SCY Community Health Worker operations including compliance with federal and state funder contractual requirements. Additionally, the Program Manager ensures the coordinated access to community-based supports and behavioral health treatment resources to meet identified client needs.
EDUCATION REQUIREMENTS: Masters in Social Work or related field with a minimum of two (2) years supervisory and management experience, or Bachelors in Social Work or related field with a minimum of five (5) years supervisory and management experience. Community Health Worker Certification from Connecticut-approved provider is preferred.

· Demonstrated ability to coordinate the delivery of community- and home-based services using the National Association of Social Workers (NASW) Standards for Care Management and other evidence-based models
· Demonstrated advanced computer literacy, ability to work in a Microsoft Windows environment, use of remote communication platforms, advanced database platforms, and utilization of mobile devices
· Demonstrated ability to work in a team approach
· Knowledge of comprehensive community and state services available within the region that provide basic needs support and behavioral health services for families of various cultural and economic backgrounds
· Skilled in crisis management/intervention with individuals and families, and familiarity with the related regional/state systems and services
· Skilled in generating and maintaining relationships with other providers for coordinated and seamless service delivery
· Knowledge of systems and conditions impacting clients’ ability to achieve well-being, including factors of race, culture, and equity
JOB EXPECTATIONS:
· Complies with SCY contractual obligations and outcomes
· Follows and implements all SCY policies and procedures, and ensures that all SCY staff are in compliance
· Represents SCY CM program professionally within the program and in the community
· Maintains a positive and respectful work attitude
· Communicates proactively with CCFSA member agency SCY Community Health Workers (CHW) and other member agency and CCFSA leadership
· Utilizes the Efforts to Outcomes (ETO) web-based data management system to inform management and supervision of CHW daily operations, including documentation of pertinent information on client interactions
· Maintains regular contact with statewide organizations to review and refine the referral process and on-going communication between agency staff
· Meets regularly with CCFSA Director of Programs and
· Provides support and guidance to Community Health Workers regarding cases in ETO on a daily, weekly or bi-weekly basis (at your discretion)
· Provides support to SCY Community Health Workers regarding community services to better support clients’ needs
· Hires and provides training for new hire and incumbent staff members
· Ensures direct coverage for SCY Community Health Workers while out of office or coverage plan with delegated covering staff information/plan of action
· Strives to fill vacancies within 30 days of notification of departing staff working in collaboration with SCY member agencies
· Assists Community Health Workers with outreach events and community collaborations to assist with referrals
· Remains current with professional credentialing requirements of self and Community Health Workers and professional best practices
· Participates in CCFSA meetings and activities as a member of the CCFSA leadership team, and/or professional group seminars, conferences, activities to increase knowledge and accepts responsibility for professional development
· Maintains flexible work schedule to accommodate CCFSA, program and client needs, including non-traditional hours as needed
SPECIFIC DUTIES and RESPONSIBILITIES:
· Participates in the interviewing and hiring of Community Health Workers (CHW) in conjunction with CCFSA member agencies
· Orients and trains new CHWs, and provides/coordinates ongoing training for all staff on child mental health screening topics and procedures
· Ensures consistency across all CHW through training, feedback and on-th-job training
· Meets regularly with CHWs for documented supervision sessions to provide direction and support for high quality, culturally and linguistically competent client service delivery
· Provides oversight and coordination of CHW caseloads and assures all referrals are timely
· Ensures that CHWs conduct outreach to build and maintain relationships with community partners and participate in community events and programs
· Reviews all client intakes and assessments with CHWs and assigns/verifies/ adjusts clients’ level of service as indicated
· Coordinates crisis intervention efforts for clients and assists with CHW referrals to community-based, emergency behavioral health services and supports
· Ensures CHWs utilize ETO case management system in a timely and proficient manner to document client interaction and decision-making
· Utilizes ETO to conduct quality checks of client electronic records to measure and track staff performance, and to assure case-record compliance with contractual, program and relevant accreditation protocols
· Provides accurate and timely feedback to member agency designated personnel regarding CHW performance and serves as co-evaluator in annual performance appraisals of CHWs
· Identifies program trends, developments and patterns that affect clients and service delivery, and provides analysis and recommendations for improvement
· Compiles information for programmatic reporting to be shared with funders, CCFSA leadership and SCY member agencies
· Attends statewide and regional meetings and trainings as required or requested
· Provides feedback to Program Director for process improvement
· Performs other reasonable duties as requested by CCFSA Director of Programs or other CCFSA leadership team members
